11/13/2023 EnchantaVaughn Approved Behavior: QMHPA arrived to the home of Ms. Broadnax. Upon arrival to the home setting, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her day. Ms. Broadnax indicated her day was well. Ms. Broadnax was well groomed and prepared for the session. Intervention: QMHPA began the session by inquiring about Ms. Broadnaxâs current thoughts, feelings, and emotions. QMHPA engaged Ms. Broadnax in a discussion to determine if there was anything that she wanted to discuss with QMHPA as it relates to stressors in her life currently. QMHPA probed Ms. Broadnax about feelings of anxiety and depression. QMHPA reiterated utilizing coping skills when negative feelings are triggered. QMHPA encouraged Ms. Broadnax to continue communicate with QMHPA her feelings as well as her journaling her thoughts provide as a healthy outlet to verbalizing her thoughts when she's feeling overwhelmed and/or having difficulty with communicating her thoughts. QMHPA educated Ms. Broadnax on relaxation techniques to learn to calm down when feeling stressed and having thoughts of negativity. QMHPA assisted Ms. Broadnax with identifying automatic negative thoughts that contributes to her negative attitude and hopeless feelings. Response: Ms. Broadnax appeared to be receptive to QMHPAâs interventions and services that were provided to her. Ms. Broadnax verbalized to QMHPA calming techniques to relax when frustrated at situations, people, and things will support her with reducing her stress levels. Ms. Broadnax reported to having too much time to herself which she thinks a lot especially at night time since she is at home. She expressed, âI have so much that I want to do with my life, but I need to get the small things in order firstâ. She continued to express, âI am working on it Ms. Vaughnâ. She expressed that she struggles with racing thoughts that sometimes she can't control her on anxiety. Ms. Broadnax expressed to QMHPA that her level of self-understanding about self-defeating behaviors are linked to feelings of depression. Plan: QMHPA will continue to teach Ms. Broadnax how to identify as well as process triggers and educate her regarding positive coping skills and interpersonal skills 1-2 times (Weekly).
